Book Reserve Request Form

Books from ASU and UNCA may be placed on reserve for a period of 3 weeks only. Faculty members are responsible for ordering the books from ABC Express one week prior to the first day they are needed.
Completed forms must accompany reserve items.

If you experience any problems with the form, please come by or call the Circulation desk at 828-227-7485.

List CALL NUMBER of library books, TITLES of personal/departmental copies - at least one book is required
