Humane care and treatment. All staff members of the Health Services will treat you with appropriate respect, dignity and consideration.
Quality Care. You have the right to a consistent quality of care.
Participate in your health care and treatment. It is YOUR body: you are a full partner in your own health care and will participate in decisions about your care and treatment.
Know your health care provider. This is why information about our staff's training, credentials, and experience is provided to you in the "Meet Your Health Professional" album in the clinic waiting area.

The specific information about your medical history and current health status contained in your medical record will not be released to any unauthorized individual without your expressed consent. This includes parents, faculty members and anyone else who may request information from Health Services without your consent or court order.
Accurate information. You will be given information, to the extent known, concerning your diagnosis, treatment and the prognosis for an illness, injury or other health related condition. This includes appropriate alternatives to care at Health Services when requested and the availability and estimated cost associated with these alternatives. You also have the right to information regarding the scope and availability of services including information about continuing health care requirements, such as return visits and required medications.
Voice your concerns. You have the right to register a complaint regarding the quality of care provided, quality of service delivered, or any other valid concern.

A referral for a second opinion. You will receive an appropriate response to any request for a second opinion regarding diagnosis or treatment.
NOTE: Any referral outside the Health Services for a second opinion cannot be covered by the student health fee.
Know about participation in research projects. You have the right to know if any aspect of your care is being used for research. You may refuse to participate in any research and it will not jeopardize your medical care.
As a Client at Health Services, you also have the responsibility to:
Be candid with your health care provider. It is important that you provide full, honest and accurate information about your illness or problem to allow proper evaluation and treatment.
Ask questions. Ask specific questions to ensure comprehension of your illness or other health problem, as well as the health care provider's recommendation for continuing care and any cost associated with that care.
Voice your concerns. The Health Service leadership team cannot know about problems unless you bring concerns about the care you receive, the course of treatment, or unacceptable interactions with the staff to their attention. Your comments, complaints, and other ideas may be submitted in person or in writing, and are both welcomed and appreciated.
Treat others courteously. The Health Services' staff members and other clients are people too; please treat them with the courtesy and respect they deserve.
Help protect your own confidentiality. Do not lend your student identification number to others. It may lead to entries in the wrong medical record or errors in treatment.
Use medications properly. Do not give medication prescribed for your needs to others. There may be a complication, such as adverse medication interactions, which could cause harm to another person.
Communicate with your health care provider. It is important to let your health care provider know if your condition worsens or if your recuperation does not follow the expected course of recovery.
Continued care. You are responsible for your continued care after you leave the Health Services. If you don't know what to do, please ask.
