Immunization information for domestic students |

Prior to registering for class, the University immunization form must be completed by all students and sent directly to the University Health Center. Students will not be allowed to register for classes with deficiencies in his/her immunization records.
North Carolina General Statute 130:A requires current immunizations.

Have your physician or Health Department clinician fill in your immunization record and update any needed immunizations that are required in Section A. This form must be signed by a MD, PA , PA -C, FNP, FNP-C or stamped by the health department. We also accept officals copies over the fax machine or via mail.

Records must be documented in black ink and all corrections must be signed. White out documents will not be accepted.
- All dates must include month, day and year of administration.
- Be certain to include your full name, date of birth and school ID number, if assigned.
- WCU Health Center administers required immunizations during Orientation
sessions. If you require additional vaccines and are unable to obtain them prior to Orientation, the Health Center can administer them for you. However, we recommend sending in all of your immunization information prior to your scheduled Orientation session in order to avoid delays in registering for classes.
- FOR STUDENTS UNDER 18: A parent or guardian signature is required in order for medical and/or counseling/psychological services to be rendered to the student.
