Patients are allowed the opportunity to file a formal grievance to voice concerns regarding their treatment or experience at Health Services. All grievances will be reported to the Director of Health Services and will undergo a thorough investigation to reach a satisfactory resolution.

Email is not a protected means of communication and would not be a guarantee of privacy regarding your confidential information.
The grievance report should include the following:
- Patient Name, Date of Birth, University ID
- Date of Service or Date of Visit
- Detailed description of grievance (who, what, when, where, why and how)
- Other attempts at resolution if applicable
Once received, the director will review the grievance and respond to the patient by the end of the next business day.
It the grievance is not resolved to the patient's satisfaction, the director will forward the grievance to the Office of Student Affairs for review.
