Prior to registering for classes and to prevent jeopardizing the status of your visa you must provide the following:

A completed immunization and a TB skin test with negative results within the 12 months preceding the first day of classes (chest x-ray required if test is positive).

Beginning Fall 2008 all international students will be required to purchase the university supported insurance plan.

Please also make note of the following requirements:

Records must be documented in black ink and all corrections must be signed.

All dates must include month, day and year of administration.
 
Be certain to include your full name, date of birth, and school ID number.
