For Faculty/Advisers/Staff
Credit by Examination	Faculty are to complete and submit this form to the Registrar's Office after the exam has been completed.
Explanation of Incomplete	Faculty are to complete and submit this form to their department head when an incomplete grade is assigned to a student.
Request for an Extension of an Incomplete Grade	Faculty are to complete and submit this form to the Registrar's Office when an incomplete grade needs to be extended for longer than one semester.
Mental Health Course Withdrawal Form	This form is part of the required packet for students to submit when processing a course withdrawal for mental health reasons.
University Group Absence	This form is to be used when planning university sponsored events.
