| 606 | Prior Year Homeless Youth | For students who were previously determined to be classified as unaccompanied homeless youth by the financial aid office. |
| 701 | Degree Plan | For students required to submit a copy of a degree plan signed by the student's advisor showing all courses still required to complete the student's degree. |
| 702 | Change of Graduation Date | For students who were scheduled to graduate Fall 2012, but will not graduate until Spring 2013. |
| 703 | Prerequisite Form | For graduate students required to take undergraduate courses. |
| 704 | Parent Refusal Form | For students whose parents refuse to provide the information necessary to complete the parent portion of the FAFSA. |
| 705 | Consortium Agreement | For students who are enrolled at another institution but taking approved coursework leading to a degree at Western Carolina University. |
| 707 | Prerequisite Form for Teacher Certification | For students who are in the teacher certification program and are required to take specific courses as prerequisites to the courses for the certification. |
| 708 | Total and Permanent Disability Certification | For students who have previously had student loans discharged because of total and permanent disability. |
