To guarantee that your appeal is heard, your appeal and all documentation MUST be received by 5:00 PM, Monday, August 17th, 2009.

Your appeal should consist of three sections as outlined below. Pay close attention to the word limit. You should not send an additional appeal letter. The Appeals Board is under no obligation to review an appeal that does not conform to these guidelines.

Write your appeal in the following space. Be sure to explain the reason you are appealing and include any pertinent information related to your academic difficulty. (Keep response to 150 words or less)
 
Explain your plan for rectifying your academic deficiencies. (Keep response to 150 words or less)
 
List the documentation you are providing. MAIL or FAX documentation to the Advising Center so that it arrives by the appeals deadline. If documentation is not provided, the Appeals Board has NO obligation to hear your appeal.
 
Note: It is the responsibility of the student to provide ALL documentation. The University and Appeals Board bear NO responsibility in gathering or requesting any documentation.
