New Interns & Students Teachers - Health Forms!

Students who will be beginning a new internship or student teaching experience Fall 2013 will need to complete a health form prior to beginning that experience.  Click here for the form.  This includes a TB test which takes several days to read, so you should plan on completing this on a Monday and returning later to have the TB test read.  Full time residential students can have this completed at no charge at the Health Center.  This form may be requested by your school system personnel office prior to the beginning of the semester.  If not, give it to the principal on your first day.  And keep a copy. Do not submit to the Office of Field Experiences. 

Students who are employed in the classroom where they work or are placed in Buncombe County Schools (not Asheville City) or are continuing Intern II in the same school system as Intern I have already completed this requirement.
