Information regarding Electronic Evidences (Complete on TaskStream):

• Individual Growth Plan (IGP) Instructions (PDF)
• Teacher Work Sample (TWS) Instructions

  • (EE3) Teacher Work Sample (TWS) - Planning (PDF)  - score of 3 - 4 on all sections required for licensure
  • (EE5) Teacher Work Sample (TWS) - Evaluation and Implementation (PDF) - score of 3 - 4 on all sections required for licensure

• (EE6) Teacher Leader Essay (PDF)  - Score of 3 - 4 required for licensure
• (EE4 & original paper copy needed with all signatures) - score of 3 - 4 on all sections required for licensure
• LEA/IHE Certification of Teaching Capacity Form (PDF version - does not need to be typed, but cannot be saved)
• LEA/IHE Certification of Teachgin Capacity Form (WORD version - has to be typed, but can be saved)
• (EE2) - Requirements vary by program.  Score of 3 - 4 required for licensure
