If access for this account is no longer required, prior to the date specified on the
initial account request form, the departmental sponsor or department head is responsible
for submitting a Consultant/Guest Access Request Form. This task should be completed in a timely manner, so that computer access can be
terminated without delay. If a termination date was not specified on the initial request
form then automatic termination of access occurs after 12 months.
