New Release of Student Information procedures in place
In accordance with FERPA, a federal law that protects the privacy of student education records, security codes are now required in order to release student information over the telephone.
To choose and register a personal security code, students must complete and submit a new Release of Information form. The forms are available at the OneStop Student Service Center or can be accessed online.
Completed forms may be submitted in person at the OneStop in Killian Annex any time during business hours. Picture ID must be shown at the time the form is submitted.
For distance learning and other students who are not able to submit a form in person, the completed and notarized form may be submitted by mail to the:
