Prior to registering for classes, University Health Services must receive completed immunization records from all students.

Records must be documented in black ink and all corrections must be signed. White out documents will not be accepted.
 
All dates must include month, day and year of administration.

Be certain to include your full name, date of birth and school ID number, if assigned.

WCU Health Services administers required immunizations during Orientation sessions. If you require additional vaccines and are unable to obtain them prior to Orientation, Health Services can administer them for you. However, we recommend sending in all of your immunization information prior to your scheduled Orientation session in order to avoid delays in registering for classes.

Prior consent of a parent or guardian is required for all students under the age of 18 for services to be rendered at University Health Services.
