M.A.T. Licensure
MAT students expecting to upgrade their licenses at the end of their programs should contact the licensure specialist, Barb Schade (227-2000; bschade@wcu.edu).
M.A.T. students will need to hand in:
- WCU Candidate Performance Appraisal (Exit Criteria) plus Advanced Competencies Teacher Performance Appraisal
- DPI Form V – Verification by Institution: Completion of Approved Education Program
- DPI Form S – Student Teaching/Interning Performance: Evaluation by NC Local Education Agency. If a student is substituting paid teaching experience for student teaching, Form S is replaced by their principal completing Verification of Teaching Experience and Candidate Performance Appraisals.
- DPI Form A – Application for a North Carolina License
- Credit card authorization (DPI Form CC), or check or money order made payable to NC Department of Public Instruction in the amount of $55.00.
