Host Teachers with Observing Students
How to Fill Out the Host Teacher Evaluation Form
Go to http://www.taskstream.com/ and enter your username and password to log in. If you’ve forgotten your password, you can click on the “Forgot Password?” link and your password will be e-mailed to you. If you need further assistance, call the TaskStream helpline at 1-800-311-5656. If you do not have a username and password, contact Dr. Lee Nickles at 828-227-3299 or lnickles@wcu.edu.

The instructions vary, depending on what class your student is enrolled in.

If your student is Observing (EDCI 231):

  1. Sign in to TaskStream (Go to http://www.taskstream.com/ and use your username and password to log in).
  2. Look for the "Host Teacher Evaluation - Observation" portfolio under the "My Programs/DRFs" heading on the main page. (If you do not see this, call or e-mail Dr. Lee Nickles.)
  3. Click on the "Evaluate" link in this row.
  4. In the box that says "Enter Name:" type in your student's name and click the yellow "Search" button. (Note: you can type in any part of the student's name; e.g., first name only, last name only, first few letters of the last name, etc.)
  5. If there is more than one student matching your search, you may have to select your student from a list. The webpage will present a list and you can then click on the name of your student
  6. Once the student is selected, their name appears in green under the search box after "Selected individual:"
  7. Click the circle beside "Evaluation (EDCI 231) " and click the yellow “Continue” button.
  8. You should see the student's name followed by a column with the heading "Evaluation" (Note: Ignore the "No Submission Required" message - this is a message to the students that they do not submit anything here.)
  9. Next, click on the yellow “Evaluate” button under "Evaluate Here."
  10. Click the green “Evaluate/Score work” button.  A new window will appear.
  11. Fill out the blanks in the form. 
  12. Finally, rate whether you believe your student meets the requirements overall for these criteria by "Meets requirements or "Does not meet requirements." (The vast majority of students meet the requirements.)
  13. For the last option under the green headers, select "Record as final and release evaluation to author now."
  14. Click the yellow “Submit” button.
  15. Done!

If your student is in Early Field Experience (PSY 321 or SPED 335/336/337):

  1. Sign in to TaskStream (Go to http://www.taskstream.com/ and use your username and password to log in).
  2. Look for the "Host Teacher Evaluation - Field Experience" portfolio under the "My Programs/DRFs" heading on the main page. (If you do not see this, call or e-mail Dr. Lee Nickles).
  3. Click on the "Evaluate" link in this row.
  4. In the box that says "Enter Name:" type in your student's name and click the yellow "Search" button. (Note: you can type in any part of the student's name; e.g., first name only, last name only, first few letters of the last name, etc.)

  9. Next, click on the yellow “Evaluate” button under "Evaluate Here."
  10. Click the green “Evaluate/Score work” button.  A new window will appear.
  11. Fill out the blanks in the form.
