______ Give agency manual to agency (Word)

______ Begin affiliation agreement (AA) process 
 
______ Send the agency Internship guidelines and AA
    
______ Get final approval of agency (on paper!)

______ Arrange for insurance    

______ Get approval of ID and register

The semester of the internship:

Week 1  

• Get hours and duties decided with supervisor
• Write weekly report on Friday
• Meet with ID

Week 2-13

• Send ID your emailed weekly reports on Friday

Week 14

• Send ID your paper, resume, weekly reports by mail
• Work must be in the WEEK BEFORE THE LAST WEEK OF CLASSES!
• Supervisor’s Evaluation form (Word)
