Alternative Routes to Teacher Licensure
Congratulations on completing your program!
To apply for your initial or “A” level license upon completion of your Lateral Entry Licensure-Only Program, Track/Phase 1 of your MAT, or entire MAT degree program without prior licenses, please complete the following:
- Form A - Application for a North Carolina License.
- Form V - Verification by Institution. Complete only top three lines.
- Form S - Student Teaching/Internship Performance. Complete top and middle portions of the form.
o Have your cooperating teacher check the appropriate box for student’s performance and sign as LEA supervisor.
o Have your principal sign as the superintendent or designee.
o BK majors: Have the licensed teacher who evaluated your teacher work sample or your cooperating teacher sign as LEA supervisor and your internship facility director or your principal signs as superintendent or designee. - If you are a lateral entry teacher and are substituting paid teaching for student teaching, Form S is replaced by your principal completing Western Carolina University's Verification of Teaching Experience and Candidate Performance Appraisals. Contact the CEAP licensure specialist to determine if this applies to you.
- Test Scores – Attach copy of Praxis II qualifying score(s), if applicable. Contact the CEAP licensure specialist to see if Praxis II is required for you. Praxis testing and registration information can be found through Educational Testing Service.
- Form E - DPI Verification of Teaching Experience forms—both professional educator and teaching assistant—can be completed if you have prior K-12 experience, one for each former employer. Mail this form, along with license application materials, to the CEAP licensure specialist to avoid additional DPI licensing fees.
