First NC Teaching License
Congratulations on completing your program!
To be eligible for an initial or A level North Carolina teacher license a student must:
- Complete one of the approved teacher education programs;
- Achieve a minimum grade of C (2.0) in student teaching and receive a public school recommendation indicating that student teaching was successful (DPI Form S);
- Receive a satisfactory recommendation on your Candidate Performance Appraisal Exit Criteria, completed after student teaching/Intern II;
- Achieve the minimum score on the PRAXIS II Tests, if applicable;
- Complete an application (DPI Form A) for licensure and file it with the licensure specialist in the College of Education and Allied Professions;
- Receive a professional recommendation for licensure by the dean of the College of Education and Allied Professions (DPI Form V); and,
- Pay the license fee required by the State Board of Education.
To apply for your teaching license, complete the following:
- Form A - Application for a North Carolina License.
- Form V - Verification by Institution. Complete only top three lines.
- Form S - Student Teaching/Internship Performance. Complete top and middle portions of the form.
o Have your cooperating teacher check the appropriate box for students performance and sign as LEA supervisor.
o Have your principal sign as the superintendent or designee.
o BK majors: Have the licensed teacher who evaluated your teacher work sample or your cooperating teacher signs as LEA supervisor and your internship facility director or your principal sign as superintendent or designee. - Test Scores Attach copy of Praxis II qualifying score(s), if applicable. Praxis testing and registration information can be found through Educational Testing Service. Contact the CEAP licensure specialist if you have questions.
